There are three forms available for the school-age assessment:
- Child Behavior Checklist (CBCL 6-18)
- Teacher Report Form (TRF)
- Youth Self Report Form (YSR 11-18)
The following cross-information syndromes were derived from all three forms:
- Anxious/depressed
- Withdrawn/depressed
- Somatic complaints
- Social problems
- Thought problems
- Attention problems
- Rule-breaking behaviour
- Aggressive behaviour
Inattention and hyperactivity-impulsivity subscales are scored from the TRF attention problems syndrome. All forms have the parallel internalising, externalising, and total problem scales and there are six DSM-oriented categories:
- Affective problems
- Anxiety problems
- Attention deficit/hyperactivity problems
- Conduct problems
- Somatic problems
- Oppositional defiant problems
