 There are three forms available for the school-age age:

  • Child Behavior Checklist (CBCL 6-18)
  • Teacher Report Form (TRF)
  • Youth Self Report Form (YSR)

The following cross-information syndromes were derived from all three forms:

  • Anxious/depressed
  • Withdrawn/depressed
  • Somatic complaints
  • Social problems
  • Thought problems
  • Attention problems
  • Rule-breaking behaviour
  • Aggressive behaviour

Inattention and hyperactivity-impulsivity subscales are scored from the TRF attention problems syndrome. All forms have the parallel internalising, externalising, and total problem scales and there are six DSM-oriented categories:

  • Affective problems
  • Anxiety problems
  • Attention deficit/hyperactivity problems
  • Conduct problems
  • Somatic problems
  • Oppositional defiant problems
